Withdrawal from alcohol or drugs is excruciatingly uncomfortable and can be potentially serious as in the case of alcohol withdrawal. Patients know that they can get immediate relief by using alcohol or drug and thus the vicious downward spiral. There is a long-held belief that forcing the patient to endure the discomfort of withdrawal will be a strong motivation not to use drugs or alcohol again. This erroneous belief is the mainstay of withdrawal treatment, euphemistically called ‘social model’ detox. Indeed, inadequate withdrawal treatment is the major cause of patients not seeking treatment and leaving treatment only to relapse.
The withdrawal symptoms are often associated with anxiety,
sleep disturbances, cramping, restlessness, increased heart rate and elevated
blood pressure. The ARCA treatment philosophy is to medically treat the
withdrawal symptoms on an immediate basis to stabilize the patient. The
medical detox is done on an outpatient basis by a staff nurse under the
directions of the physician. The patient returns home with written instructions
on the medications to be taken for the symptoms associated with the withdrawal.
The patient returns to the clinic the next day and the treatment is continued
to include medications to attenuate cravings for the drug or alcohol. Most
patients do not need detoxification beyond 3-5 days, but this is the most
critical component of treatment for long-term recovery.
