one of the three methods to complete your paperwork.
Complete Online
Completing your forms online will save you time on the day of your appointment. ++Surveys are HIPAA compliant and secure.
Download / Print
Download the new patient forms listed below and fill them out prior to coming to your first appointment. You can either drop off this paperwork at ARCA ahead of time or come 15-20 minutes early for your appointment to submit the paperwork to our Check-In staff, so it can be entered into the system before you meet with the provider..
Arrive Early
If the other two methods aren't a good fit for you, feel free to arrive early on the day of your appointment to complete the paperwork in person.
